IMPORTANT INFORMATION FOR NEW PATIENTS
Every patient must fill out and sign three forms before the start of the first appointment. If you don't bring them or don't arrive at least half an hour before the scheduled appointment to fill them out you will likely need to reschedule your appointment for another time.
The forms can be downloaded from the links provided in the table below. If you do not have access to a printer or would rather fill them out at the office, please arrive 1/2 hour before your scheduled appointment. Wear or bring loose clothing. If you have an extremity problem (ex, knees), bring or wear shorts.
When you arrive to the Casa Cristal, please knock so that I know you have arrived. Please arrive 15 minutes before your scheduled appointment. If you are late you may have to reschedule for another time.
